⋅Phthirus pubis (crab louse)

⋅인간이 유일한 보균자이다.

⋅머릿니보다 생존기간이 더 짧다. (24시간)

⋅대부분 음모와 사타구니에 존재하지만 가슴, 겨드랑이, 속눈썹 혹은 안면의 털에서 관찰되는 경우도 있다.

⋅전염은 직접적인 성접촉뿐만아니라 성접촉이 아닌경우에도 일어날 수 있다.1

===임상소견===

증상 및 징후2

⋅털에 들러붙어 있는 서캐 (nits) 또는 이 (lice)가 관찰된다.

⋅가려움, 긁힘, 홍반, 피부자극과 염증 모두 이에 물린 반응으로 나타난다.

⋅광범위한 감염은 가벼운 발열과 권태감을 유발할 수 있다.

⋅긁으면 2차 세균성 피부감염으로 이어질 수 있다.

A Permethrin 1% 로션을 이환된 털에 바른 후 10분 후에 씻어낸다.
A 0.33% pyrethrin-piperonyl butoxide 샴푸를 이환된 털에 바른 후 10분 후에 씻어낸다.
A Lindane 1% 로션 또는 크림을 이환된 털에 바른 후 4분 후에 씻어낸다.
D Crotamiton 10% 연고를 이환된 털에 바른다.
D 의복 및 침구류는 50℃ 이상의 뜨거운 물로 세탁하거나 드라이클리닝을 한다.

===추적관찰===

D 1주일 후나 필요하다면 2주 뒤에 털에 들러붙어 있는 이를 다시 찾아본다.
D 반드시 성 파트너도 함께 치료를 받아야 하며, 감염이 확인된 환자와 동시에 치료를 시행하는 것이 권장된다

===검사대상의 선택===

⋅성매개 감염이 의심되는 환자를 대상으로 시행한다.

⋅진단은 항상 전형적인 임상증상을 토대로 이루어진다.

 털에 들러붙어 있는 이 또는 서캐를 직접 또는 현미경을 통해 관찰한다.

===성충 사면발이 (adult lice)와 서캐에 대한 주의 깊은 검사===

⋅털에서 서캐가 있는 딱지 주변을 찾는다.

⋅딱지가 성충 사면발이 일 수도 있다.

⋅서캐는 털에 붙어서 쉽게 떨어지지 않는다.

===검체수집과 실험실 진단===

⋅확실한 진단이 어려운 경우는 용기에 이나 서캐를 담아 현미경 검사를 한다.

==치료==

===치료의 시작===

⋅사면발이의 치료에 대한 비교관찰연구는 드물며,3치료지침의 대부분은 머릿니에 대한 연구를 바탕으로 한다.4-6따라서 머릿니에 적용되었던 연구결과가 사면발이에서는 다르게 나타날 수 있다.7,8

⋅따라서 만약 어떤 박멸제제에 효과가 없다면, 다른 치료제제를 고려한다.9

⋅진단된 환자는 첫 내원시 바로 치료를 시작하는 것이 좋다.

⋅3-7일 후 반복할 수 있으며, 필요하다면 1주일 후에 반복할 수 있다.

⋅가려움증은 치료 후에도 몇 일 또는 몇 주 동안 지속될 수 있다.

⋅증상호전이 없을 경우에는 1주 후 다시 치료 할 수 있다.

⋅가려움증은 hydroxyzine이나 diphenhydramine 과 같은 항히스타민제제 또는 경한 국소 코르티코스테로이드로 조절할 수 있다.2

===치료약물의 사용법===

⋅Permethrin,10pyrethrin,lidane2,10등이 사용된다. (근거수준 1)

⋅또한 우리나라에서는 crotamiton 연고도 판매되어 사용되고 있다. (근거수준 4)

⋅병변부위를 씻고 이 박멸제제 (크림, 로션 또는 샴푸)를 지시대로 바른다.

⋅이환된 음모나 몸의 모든 털이 있는 부분에 박멸제제를 바른다.

⋅콧수염 또는 턱수염에 감염이 있는 경우 동일하게 박멸제제를 바른다.

⋅Gamma benzene hexachloride (lindane)는 신경독성을 일으킬 수 있다. 사용 전 교육은 독성의 위험을 최소화 할 수 있도록 주의 깊게 이루어져야 한다. 2세 이하의 아이, 임신 중, 수유 중 또는 광범위한 피부염을 가진 환자에서는 금기이다.11

 Lidane은 2세 이하, 임신 또는 수유 중, 광범위 피부염 환자에서 금기이다.

===치료 중 의복 및 침구류 관리===

⋅치료시작 시 의복과 침구류는 사면발이가 감염되지 않은 새것을 사용한다.

⋅의복 및 침구류와 같은 매개물은 뜨거운 물 (50℃)로 세탁하거나 드라이클리닝을 해야 하며 세탁물의 온도가 50℃ 이하인 경우 완전한 이의 박멸을 보장할 수 없다.12

⋅침대이불은 진공청소를 한다.

 의복 및 침구류는 50℃ 이상의 뜨거운 물로 세탁하거나 드라이클리닝을 한다.

===임신 또는 수유부의 사면발이감염===

⋅임신부에서 permethrin은 안전한 것으로 보인다.13(근거수준2)

C 임신 또는 수유부의 사면발이감염은 Permethrin을 사용한다.

===속눈썹에 발생한 사면발이감염===

⋅속눈썹의 이에 의한 감염증은 pyrethrin이나 lindane으로 치료해서는 된다.2

⋅족집게를 이용하여 속눈썹에 들러 붙은 이를 제거하거나 폐쇄성 안연고 (occlusive ophthalmic ointment)를 눈꺼풀 가장자리에 10일 동안 1일 2회 바른다.

⋅또는 면봉을 이용하여 속눈썹에 permethrin 1% 로션을 바르고 10분 후 씻어 낸다. 이 때 permethrin은 눈자극은 없으나 눈을 감은상태에서 치료를 시행한다.

⋅B형 간염바이러스에 관한 면역이 이루어지지 않았다면 예방접종을 권장한다.
