===Subinguinal varicocelectomy===

*Patient position : Supine and in slight reverse Trendelenburg position
# External ring에서 Langer’s line을 따라 3cm transverse incision
# Subcutaneous layer를 지나 Scarpa fascia까지 도착
# Index finger를 이용해 Scarpa fascia를 blunt dissection 하고 subinguinal spermatic cord를 관찰
# Babcock clamp를 이용해 cord를 들어올리고 Kittner dissector를 이용해 cremasteric m.과 cord를 분리 
# Perforating external spermatic vein을 4-0 silk로 tie하고 절제
# Testis를 절개창 밖으로 꺼내어 확장된 gubernacular vein을 4-0 silk로 tie하고 절제하고 testis를 scrotum 안으로 위치 시킴
# 설압자를 spermatic cord 밑으로 넣고 external spermatic fascia를 절개
# Gonadal artery의 pulsation을 확인하고 gonadal artery와 vas deferens, lymphatics를 보존하고 dilated spermatic vein을 4-0 silk로 tie하고 절제
# Scarpa fascia와 subcutaneous layer 그리고 skin을 layer to layer로 봉합하고 수술을 마침

===Inguinal varicocelectomy===

*Patient position : Supine and in slight reverse Trendelenburg position
# Symphysis pubis에서 2finger 위 정도 level에서 scrotum의 lateral aspect에서 시장해 inguinal canal쪽으로 oblique하게 3cm 크기로 절개
# Subcutaneous layer를 지나 scarpa fascia까지 도착
# Scarpa fascia를 절개하고 external oblique aponeurosis를 절개하여 inguinal spermatic fascia를 노출 시킴 
# Spermatic cord를 thumb과 forefinger를 이용해 들어 올리고 설압자를 cord 밑으로 넣음
# Spermatic fascia를 절개하고 vas deferens와 artery를 촉지하여 분리 시킴
# Artery와 vas deferens, lymphatics를 보존하고 dilated spermatic vein을 4-0 silk로 tie하고 절제
# External oblique aponeurosis를 4-0 synthetic absorbable material로 suture 
# Scarpa fascia와 subcutaneous layer 그리고 skin을 layer to layer로 봉합하고 수술을 마침
