Post-op order

1. Vital sign - q 15min×2, 30min×2, 60min×2, if stable q 4/6/8 hrs (ICU 환자는 q 1hr) 2. Activity - BR or ABR 3. 식이처방 - Major OP: NPO until next order or gas-out - Minor OP: NPO for 6hrs-SOW-SD 4. I/O (Input/Output) - ICU care 환자는 q 1hr - q 4/6/8hrs 5. B/W (Body Weight) 6. BST q 4times (당뇨환자의 경우) 7. 간호처치 - Spinal anesth.: ABR c head flat for 8hrs - General anesth.: EC & DB - I/O balance 유지가 중요한 환자의 경우 Urine output 시간당 100cc 미만 시 notify to R1 1. 5% DS 1L or H/S 1L or 소아는 DNK3 (5% Dextrose+Na77meq+K30meq) 500ml - glucose가 포함된 수액 - 특히 TURP (or TURB) 수술은 TUR synd 발생 가능 성 있어 NS 1L 2. IV antibiotics 3. mucolytics-전신마취한 경우 4. transamin/botropase-출혈이 심한 경우 5. H2 blocker (curan)-stress induced gastric ulcer 예방 목적 6. 진통제 7. X-ray - KUB: stone 환자 및 D-J stent insertion - Abdomen flat: peritoneum을 열고 수술한 경우, 복강경 수술, 경우에 따라 retroperitoneum을 열고 수술한 경우 → 가능하면 upright도 같이 - Chest PA or AP: 수술 시간이 길었던 전신마취 환자 8. Lab. - CBC and WBC differential count - Chemistry Panel (9test)삭제 - Electrolyte Panel II (4test)삭제 - Blood Gas Analysis (BGA): 장시간의 전신마취 ☞ bleeding이 많았던 수술, TUR 수술, Major OP 시 에 시행, URS 및 소아는 굳이 post OP Lab.을 할 필요 는 없다. ☞ Chemistry Panel 대신 간단하게 Urea Nitrogen (BUN)/Creatinine ☞ Rationale가 있게 Lab. 검사를 할 것. 9. 기타 - Stone Analysis정성 : stone 환자에서
