- Patient position : Lithotomy position
1) 18Fr. Foley catheter를 삽입하고 10cc ballooning을 시행하여 방광내 소변을 제거한다.
2) Ring retractor를 이용하여 외음순을 lateral side로 당겨서 urethra, vagina를 노출시킨다.
3) anterior vagial wall에 urethral meatus의 1cm 하방으로부터 1.5-2.0cm의 vertical incision을 시행한다.
4) 양측으로 Vaginal epithelium을 periurethral fascia로부터 dissection하여 (inferior pubic ramus까지) index finger가 통과하여 needle 과 접할 수 있도록 한다.
5) Obturator foramen의 internal edge을 확인한 후 adductor longus tendon base에서 양측의 genitofemoral fold에 clitoris level 정도에서 0.5-1cm의 skin incision을 시행한다.
6) Bladder가 비워진 상태임을 재확인하고 needle을 skin incision으로 통과시켜 obturator foramen의 internal edge를 따라 vaginal incision방향으로 통과시키고 Index finger를 이용하여 needle tip과 접하도록 하여 vaginal incision까지 도달하도록 유도한다.
7) 한쪽의 needle이 제대로 위치하게 되면 다른 needle도 같은 방식으로 시행한다.
8) Vaginal incision으로 나온 needle tip에 mesh를 attachment시키고 mesh가 꼬이지 않게 flattening한 후 insertion시 pathway를 따라 needle을 retraction한다.
9) Mid-urethra와 mesh사이에 3-5mm의 small space를 남겨 tension 없이 mesh를 위치시킨다.
10) Sheath를 양쪽 끝에서 잡아당겨 removal하고 skin incision밖으로 남겨진 mesh를 trimming한다.
11) Cystoscopy를 시행하여 bladder injury가 없음을 확인하고 foley catheter를 삽입한다.
12) Vaginal incision과 양쪽의 skin incision을 closure 한다.
13) Vaginal packing을 시행한 뒤 수술을 종료한다.
